Bernadine H. Olsofka, 77, passed away on Saturday, September 6, 2025, at Homewood Health Campus in Lebanon, Indiana. She was born on September 18, 1947, in Chicago, Illinois; a daughter of the late Stanley Kuklewicz and Harriet Kurpiewski.
She was married to Joseph P. Olsofka and he preceded her in death.
Bernadine was a member of St. Joseph Catholic Church in Lebanon. She cherished her Goldendoodle, Ginger, and enjoyed spending time with her beloved family.
Survivors include her son, Jason (Casey) Olsofka; grandchildren, Kristofer (Itati) Olsofka, Matthew (Zoie) Olsofka, and Stephanie Olsofka; and great grandchild, Link Olsofka.
In addition to her parents and husband, Bernadine was preceded in death by her sister Jeanne Stevens.
A Memorial Mass will be held at 11:00 a.m. on Saturday, September 27, 2025, at St. Joseph Catholic Church, 319 East South Street in Lebanon, Indiana. Myers Mortuary and Boone County Crematory have been entrusted with her care.
