At approximately 2:10 p.m. on May 18, 2026, officers with the Lebanon Police Department responded to a serious crash with entrapment at the intersection of State Road 32 and County Road 400 West. The collision involved a passenger vehicle, a semi tractor-trailer, and a dump truck.
According to the preliminary investigation and witness statements, the crash may have been caused by a passenger vehicle entering SR 32. Authorities emphasized that the investigation remains ongoing.
The driver of the dump truck was trapped inside the vehicle and had to be extracted by emergency crews before being transported to an Indianapolis-area hospital. The driver, identified as Jaheim Miller, 22, later died from his injuries.
Occupants of the passenger vehicle and the semi tractor-trailer were transported to a local hospital with non-life-threatening injuries.
The crash scene forced the closure of this section of SR 32 for several hours while investigators processed the scene and crews removed the damaged vehicles.
The investigation is being conducted by the CRASH Team with assistance from several agencies, including the Boone County Sheriff’s Office, Lebanon Fire Department, Center Township Fire Department, Boone County EMS, Whitestown Police Department, Zionsville Police Department, and Zore’s Towing.
Officials remind the public that all individuals involved are presumed innocent unless proven guilty in a court of law.
