*Update* 3 Vehicle Traffic Accident Results in Fatality

On November 18th, 2025, the Boone County Sheriff’s Office would like to provide an update on the accident involving a military Humvee that took place on November 14th.

The driver of the Humvee has been identified as Izaya Phillips, 21, of Ingalls, IN. The three passengers inside the Humvee were identified as Nicholas Reinhart, 27, of Washington, IN; Luke Richardson, 23, of Carmel, IN; and Terry Frye, 23, of Lafayette, IN. Mr. Frye was pronounced deceased on the scene, and the other passengers sustained injuries and were transported to a local hospital.

The driver of the white Subaru was identified as Veronica Webster, 51, of Independence, KY. Ms. Webster
was treated on the scene.

The driver of the car hauler truck was identified as Goderdzi Gujabidze, 56, of Brooklyn, NY. Mr.
Gujabidze was treated on the scene.

During the initial investigation of the accident, all non-injured drivers were transported by law
enforcement to a local medical facility for a blood draw and post-accident interview, in accordance with
department policy. Due to the language barrier with Mr. Gujabidze, he was transported to the Boone
County Sheriff’s Office afterward to meet with a translator.

During the course of this initial investigation, our department was notified by Immigration and Customs
Enforcement (ICE), that they had placed a hold on Mr. Gujabidze. The Boone County Sheriff’s Office
detained Mr. Gujabidze in our facility in accordance with department policy. The Boone County Sheriff’s
Office recommends contacting ICE directly for specific information regarding the status of Mr. Gujabidze.
The actual cause of the accident is still under investigation as we continue to work alongside the National
Guard and Federal Motor Carrier Safety Administration.
